YMP Volunteer Application

Thank you for your interest in volunteering for the Youth Music Project! Volunteers play a critical role in helping the Youth Music Project reach some of our community's most underserved youth. YMP volunteers do everything from serving as teaching assistants to helping coordinate overall program activities. If you are interested in helping out, please fill out this form and we will get in touch with you shortly if it looks like a good fit. You will be asked a series of questions and will be given the opportunity to upload your resume or CV. The entire application should take about 10 minutes to complete. Please note that volunteers are asked to commit to volunteer at YMP for a minimum of 6 months and all volunteers must pass a background check. Thanks in advance!

I understand that any misrepresentation of information in this application shall be considered sufficient reason for withdrawal of an application with the Youth Music Project or for termination of my volunteer experience, if Youth Music Project selects me. Should I be selected, I understand that my volunteer experience will be “at will”; this means that either I or the Youth Music Project may end our volunteer relationship at any time and for any lawful reason.

I hereby authorize the Youth Music Project or its authorized representatives bearing this release or copy thereof to request and obtain information pertaining to my employment history and criminal background including but not limited to information concerning criminal or civil court records, character, work habits, performance experience and reasons for termination of past employment. I understand that Youth Music Project may request authorization to conduct further background checks if necessary and that such additional authorizations may be required in order for my application to be considered.

I hereby direct all parties who receive a request from Youth Music Project for the information described above to release such information at the request of the Youth Music Project or its employees or agents.

I hereby release and hold harmless all persons or entities, including without limitation the Youth Music Project, the Oregon Department of Education, any agencies processing the background checks authorized herein, and any affiliates, partners or clients of the same, from any and all liability whatsoever arising from or relating in any way to my application for employment with the Youth Music Project or the action that the Youth Music Project may take based on this application, including any background checks. The release above also applies to the employees and agents of all of the released parties.
